This privacy statement covers MYHEALTHFINDER.ORG
and sets forth the terms and conditions for using MYHEALTHFINDER.ORG iOS and Android platform applications (“Apps”) and (“Website” together with App, the “Site”) which was developed as a collaborative effort by various nonprofit organizations and institutions funded through grants to the TX A&M Health Science Center School of Public Health. HEALTHYSURVIVORSHIP.ORG complies with the U.S.– EU Safe Harbor framework and the U.S.– Swiss Safe Harbor Framework as set forth by the U.S. Department of Commerce regarding the collection, use, and retention of personal information from European Union member countries and Switzerland. HEALTHYSURVIVORSHIP.ORG is intended for use within the United States. HEALTHYSURVIVORSHIP.ORG adheres to the Safe Harbor Privacy Principles of notice, choice, onward transfer, security, data integrity, access, and enforcement. Upon request and within 30 days, HEALTHYSURVIVORSHIP.ORG will grant individuals reasonable access to personal information that it holds about them. Privacy Matters
HEALTHYSURVIVORSHIP.ORG understands that health is a very personal, private subject, and we want you to feel as comfortable as possible visiting our website and using its services. This statement sets forth our Privacy Policy and advises you about the information we collect, how we collect it, how we secure and protect it, and what choices you have about how that information is used.
HEALTHYSURVIVORSHIP.ORG knows that privacy and security are important to you and to all of our users. Earning and keeping your trust and respect is of primary importance to us. We pledge to protect your privacy and keep your personal information secure and confidential. We will not give, sell or distribute your information to any other organization for promotional purposes. Be assured that the personal information you share with HEALTHYSURVIVORSHIP.ORG is kept strictly confidential and fully secure unless you, the user, specifically request otherwise. It can only be accessed and used by members MYHEALTHFINDER team and all data is de-identified prior to sharing with MYHEALTHFINDER team members.
Information We Collect
We collect information from you in three distinct areas:
- Create a Username and Password
- Complete a HEALTHYSURVIVORSHIP.ORG assessment with associated scores and recommendations.
- Contact Us Emails
The information we collect in these areas is limited to the information that you choose to share with us. The information is stored in the HEALTHYSURVIVORSHIP.ORG database until you decide to delete it.
- To protect your privacy, our Create a Username and Password form can be filled out anonymously, meaning that no personal or identifying information is required. However, email addresses are required to create the assessments. You may use any of the publicly available resources on the site without creating a login or password. User names and passwords are required for use with the Healthy Survivorship Assessment. All passwords and login information is secure, password-protected environment.
- By creating a username and password and providing your email address, you grant HEALTHYSURVIVORSHIP.ORG the right to receive, transmit, monitor, retrieve, store, and use this information in connection with the operation and maintenance of this feature. In accordance with the provisions of this Privacy Policy we will not give, sell, or distribute this information to any other organization. De-identified information, which will not include any user names or emails, may be used for research and research publication by the HealthySurvivorship.org team members.

How We Protect Your Information
HEALTHYSURVIVORSHIP.ORG keeps all your personal information confidential on a password-protected system. No other user can view or change your information without your Username and Password. HEALTHYSURVIVORSHIP.ORG puts you, the user, in control of your personal information. Your information is saved until you edit, update or delete it. HEALTHYSURVIVORSHIP.ORG wants your personal information to remain as secure as reasonably possible. That is why we have a system at HEALTHYSURVIVORSHIP.ORG that combines leading technical safeguards and a code of conduct for those employees that are permitted to access our users’ personal information.
What We Do About “Cookies”
“Cookies” are small pieces of information that are stored by your browser on your computer’s hard drive. HEALTHYSURVIVORSHIP.ORG does not store information about you in cookies that we can find and use later. Therefore you can feel secure that unless you choose to share information with HEALTHYSURVVIVORHSIP.ORG in one of the four areas described earlier, we will not collect any personally identifiable information about you without your consent.
HEALTHYSURVIVORSHIP.ORG may employ what are known as “session cookies” that allow you to move around the website. Those session cookies are deleted when you leave HEALTHYSURVIVORSHIP.ORG. They are not used to track and store personal information about you or your Internet use.

Log Files
As is true of most web sites, we use a third party tracking–utility partner to gather certain information automatically and store it in log files. This information includes Internet protocol (IP) addresses, browser type, Internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and clickstream data.
We use this information, which does not identify individual users, to analyze trends, to administer the site, to track users’ movements around the site and to gather demographic information about our user base as a whole.
Except as set forth in this Privacy Policy, we will not disclose to unrelated persons or entities any information you provide to HEALTHYSURVIVORSHIP.ORG that could be used to identify or contact you (“personal information”). The Create a Username and Password and user profile systems are powerful storage tools that we hope you will find useful. It is your sole responsibility to: (1) control the dissemination and use of usernames and passwords; (2) authorize, monitor, and control access to and use of your usernames and passwords; and (3) promptly inform HEALTHYSURVIVORSHIP.ORG of any need to deactivate a username or password. You grant HEALTHYSURVIVORSHIP.ORG the right to receive, transmit, monitor, retrieve, store, and use your personal information, including information that may be privileged and confidential under applicable state and federal laws, in connection with the operation of or maintenance of your user profile and account. HEALTHYSURVIVORSHIP.ORG cannot and does not assume any responsibility or liability related to the accuracy or inaccuracy of the information you provide to us. HEALTHYSURVIVORSHIP.ORG also cannot and does not assume any responsibility or liability related to your or third parties’ use or misuse of information transmitted or received using HEALTHYSURVIVORSHIP.ORG.
Privacy of Children
HEALTHYSURVIVORSHIP.ORG does not collect, maintain, store, or disclose personal information about children. Users of the site are asked to be 18 years of age or older.
Your Consent
By creating a username and password (registering) and filling out a HEALTHYSURVIVORSHIP.ORG assessment, you grant HealthySurvivorship.ORG the right to receive, transmit, monitor, retrieve, store and use your personal information, including information that may be privileged and confidential under applicable state and federal laws, in connection with the operation or maintenance of these features.
HEALTHYSURVIVORSHIP.ORG cannot and does not assume any responsibility or liability related to the accuracy or inaccuracy of the information you provide to us. HEALTHYSURVIVORSHIP.ORG also cannot and does not assume any responsibility or liability related to your use or misuse of information transmitted or received on the site.
HEALTHYSURVIVORSHIP.ORG has the right to suspend or eliminate your right to use this site and to remove any information that you have placed on our site. HealthySurvivorship.ORG may also change this Privacy Policy at any time by posting revisions on our website. Your use of the site constitutes acceptance of the provisions of this Privacy Policy and your continued use of the site after such changes are posted constitutes acceptance of each revised Privacy Policy. If you do not agree to the terms of this Privacy Policy or any revised policy, please exit this site immediately.
